Because I am an immigrant, and my American dream is a long-lasting mission for knowledge and skills to serve and alleviate suffering. As a PA I will contribute to the society that once sheltered me in a moment of need. I choose PA because it feels me right at that moment based on reasons such as flexibility to change specialty, high-quality patient care, and teamwork. The awe of discovering the human body, the honor of being trusted to give advice, the gratitude for helping someone through a difficult illness propels me to make my patients heathier.

WHAT I LEARN FROM MY PATIENT.
What my patients say is extremely important to my medical success and professional development. That’s why I do my very best to guarantee that each one of them is happy with the health treatments and services I provide. Take a look at what my past patients teach me.

Sometimes patients are the best teacher for healthcare provider. In my case one of the patients teach me a beautiful life lesson. It was early morning and I entered patient’s room for morning round and brief history taking. I took his permission to talk and the gentleman was so polite that he granted me permission to ask questions even though he was taking his breakfast. During the interview session I found that he has very extensive list of medical problems. I expressed my empathy to the patient with words, but I think he feels my sadness for him behind my mask. He politely asked me if I can talk with him five more minute. I agreed on that and he asked me, have you ever had enjoyed barbequed corn and fibers are stuck in your tooth? I was not prepared for the random question, but I answered, yes, sir! He replied isn’t it amazing! When something stuck in our tooth, the tongue just keeps going to that tooth until unless that stuff is out of that tooth, it just keeps going there. He asked me again, did I know the reason why the tongue is going only to the one tooth where something is stuck not the other 31 teeth! I was answerless because I tried to find an answer from medical perspective. He said, there is lot of good things in our life to focus but our notorious mind always stuck in a certain area of our life where we have problems. So many good things happening in so many different areas of life where we have no problem or issues. We need to focus that is good happening in the life and deal with the problem in the life. I was so much surprise by that patient’s attitude. Let me tell you, the patient has medical history of breast cancer which metastasized to lung and liver. Moreover, he had hypertension, hyperlipidemia, thyroid nodule, and dilated cardiomyopathy. Still instead of being depressed and loosing hope, he is enjoying his life. He was admitted for CABG surgery in the hospital. He has no complain or questions, but he had vision to live his life more beautifully. Yes, my patient taught me a good life lesson: attitude of gratitude.

Although I only worked with Dhruvit for less than one year, his personality and qualities stood out exponentially. Dhruvit is very eager to learn, and was always willing to go above and beyond to fully understand the cardiac disease processes of the patients we were evaluating and treating. His work ethic is incomparable to others, as he surpasses every expectation of a perfect student, exemplifying the passionate and determined healthcare provider he will grow into in his future career. His behavior revealed his desire and passion to develop a career in healthcare. He is extremely intelligent, and I was consistently impressed with his knowledge base. Dhruvit is dependable, loyal, and responsible, as he followed through on any project he was given.
Dhruvit also had great rapport with my staff, supervising physicians and with our patients, highlighting his admirable interpersonal skills. Dhruvit had an infectious attitude during his rotation, demonstrated by his smile and ability to bring optimism into any room or situation. Not surprisingly, he was very kind, respectful and professional with patients. It was easy to see Dhruvit’s empathy and compassion during each patient encounter – they thoroughly enjoyed getting to know him and see him grow as a potential future healthcare provider.

I am an attending emergency physician at Cleveland Clinic Mercy Hospital where I currently served as the assistant medical director of the emergency medical department and president of the medical staff. Additionally, I have served as the chair of our hospitals medical staff performance improvement committee and have been involved in bedside teaching of medical residents and PA students for the last 15 years.
I am enthusiastically writing this later for Dhruvit Patel. I have had the opportunity to work clinically with Dhruvit during his emergency medicine clerkship. Understandably, given his background as a practicing physician in India, he possessed a medical fund of knowledge and clinical decision-making that far surpasses any PA student I have worked with over the years. A NEW MEDICATION SHOWS GREAT PROMISE: RELIEF IS IN SIGHT FOR CHRONIC MIGRAINE SUFFERERS

Migraines may not be fatal, but they can be debilitating enough to drastically affect a patient’s quality of life. There are numerous people in the world who are brilliant and successful but had to steer far away from work because of migraine. It also affects society through direct and indirect medical costs. All healthcare providers encounter patients with chronic headaches. Patients with chronic headache seen by primary care are typically referred for management to secondary care and conjure a large proportion of patients in specialist headache clinics. Chronic migraine typically develops after a slow increase in headache frequency over months to years. Chronic migraine sufferers are those who continue to have headaches despite taking two or more antimigraine medications. A systematic review of literature was conducted using peer-reviewed headache journal publications, the US Food and Drug Administration trials and other studies utilizing randomized, double-blind, placebo-controlled designs. The goal of the review was to create a practice guideline identifying how prescribers should approach the use of erenumab, an exciting, novel medication on the market. Findings conclude that erenumab reduces the number of migraine days and holds promise as a safe and effective drug of choice when other preventive measures have failed.


EVOLUTION OF OUTCOME AND COMPLICATIONS IN TAVR WITH LOW AND INTERMEDIATE SURGICAL RISK PATIENTS: WHERE WE STAND TODAY

Aortic stenosis is the most common valvular disease worldwide which is characterized by progressive thickening, fibrosis and calcification of the leaflets leading to restriction and valve obstruction. If not treated, patients with severe aortic stenosis can die from heart failure in as little. The new approved technique called transcatheter aortic valve replacement (TAVR) is a less invasive aortic valve replacement technique to replace aortic valve through venous access. The technique is recommended based a thorough examination of data confirming that these devices are safe and effective in a larger population. Treatment with TAVR, like open-heart valve replacement surgery, carries the risk of significant consequences including death, stroke, acute renal injury, myocardial infarction, hemorrhage, and the necessity for a permanent pacemaker. Today, there is enough proof assisting TAVR in high surgical risk aortic stenosis. However, currently TAVR has been prolonged for low to intermediate surgical risk severe aortic stenosis patients and applicable clinical evidence continues to be emerging, leaving a few uncertainties. This systemic review of literature provides a background of the TAVR procedure, reviews relevant research, and highlight contemporary approaches to these complications as well as the current use of TAVR in intermediate and low risk severe aortic stenosis patients.
